Mate, I bank with HSBC so when I asked for a U.S dollar account they said that I had to be a premier account holder, which I was nt. Then not so long ago I had an account revamp and when the manager asked how much I earn and why does the money come from Barclays each month to pay the mortgage and house bills I quite happily told him that I did nt meet their criteria and that Barclays Bank are quite good...with a smug look on my face.

I'm not a Premier account holder at HSBC and I have a USD account with a VISA debit card. I've had the account since 1998. They usually try that shit when they are trying to get you to hold all your money with them. Bank managers have a lot of discretion, find one who isn't a prick and will make the phone call. :) Or, open an offshore account with HSBC Jersey (I think it's 5k minimum now, was 2k when I did it) and tell them you need to pay in USD and don't want an exchange rate hit and they'll give you a shadow USD account with a card.
