Excellent read, I envisage some fierce debate on some of the medical forums!

Will it lead to members of the public claiming that their loved ones didn't receive adequate care or that medical staff didn't give Uncle Bob a chance to jump start himself before following his wishes as recorded on his donor card...will it lead to a reduction in the harvest of viable organs etc...

Lots of questions & not enough answers at this point me thinks!
 
Good read, but more a non-technical update on what's already happening. There has been routine cooling post cardiac arrest since the 2010 guidelines, and many centres had been at least trialling it before then. In my experience, I've not seen a change in "did Uncle Bob get given a chance" type queries, but have seen "what are you doing, Uncle Bob wouldn't want this" when we have been trying to explain that we can't tell yet how Uncle Bob will come out of this as we've cooled him and are going to rewarm him.
There have been no changes in the laws and ethics around organ donation, but there is a case for arguing that for out of hospital cardiac arrests who now go to a PAMI centre, there may actually be an increase in those who are eligible for donation after either brain or circulatory death instead of them simply dying in the ED soon after arrival at hospital and going for Coroner's post-mortem.
